Can the primary care setting be a home-based primary care? Can it be a specialty-related primary care program, such as geriatric or HIV primary care?

Home-based primary care is within scope for the primary care setting. Specialty-related primary care may be appropriate, but primary care must be the focal setting, and the population must meet the criteria for multiple chronic conditions as outlined in the PFA.
