Must there be a pharmacological agent proposed to be studied or can there be two non-pharmacological interventions proposed?

Yes. For the purposes of this PFA, at least one of the interventions proposed for comparison must be a pharmacological agent or pharmacological treatment regimen as determined by current clinical practice guidelines. The comparison may include another non-pharmacological intervention to relieve symptoms but at least one of the comparisons must be made directly to a pharmacological intervention.
